Thank you for contacting Oberlin Road Pediatrics. You will receive a copy of your request form via email as confirmation that your request has been received. If you do not see the email in your Inbox, please check your Junk and Spam folders.

Once you have been added to the waitlist, we will reach out to you to start the onboarding process for your family. One of our patient coordinators will reach out to you to get you onboarded.

We will provide a new patient packet that will need to be filled out for each of your children. You will need to obtain a copy of each child's medical record before your child(ren) can be registered and seen.
